Disclosure of rental properties on City form

What are the requirements regarding disclosing rental property on the City Financial Disclosure Form?

Filers are required to disclose any  real  estate  in which the filers have an interest that was sold to the  City, purchased  from the  City, leased  to or from the  City, or subject to any condemnation proceeding by the City in the previous calendar year. If a filer receives rental income of more than $500 during the calendar year, that must be disclosed as well.
